Free Testosterone

Measured in pg/mL — assay specific

The small unbound slice that can actually enter cells and do the work. Often the truer measure of how you feel.

What the numbers mean

These are the ranges BioForge works to. They are not the same as a lab's reference range, which is built from the general population rather than from people who feel well.

Low
<6.5 pg/mL (direct) · <5.0 ng/dL (equil.) · %Free <1.5%
Optimal
15–25 pg/mL (direct) · 12–20 ng/dL (equil.) · %Free 2.0–3.5%
High
>25 pg/mL (direct) · >21 ng/dL (equil.) · %Free >4.2%

What moves Free Testosterone

Driven almost entirely by SHBG. Also moved by albumin, insulin resistance, thyroid status, liver health, and estrogen. Units are not interchangeable between methods. Labcorp reports the direct immunoassay in pg/mL and the equilibrium method in ng/dL — a 15 on one is nothing like a 15 on the other. Always read against the range printed on your own report.
